About The Role

We're seeking a seasoned leader to drive the implementation of our Population Health strategy. This pivotal role will focus on transforming operational quality and safety across our organization.

The ideal candidate will have experience in managing complex programs or change initiatives in a healthcare setting, with the leadership capability to support and guide our Population Health team.

Key responsibilities include:

  • Developing and implementing population health strategies that align with our organizational goals
  • Managing physical, human, and financial resources to achieve optimal health outcomes
  • Collaborating with stakeholders to identify and address health inequalities
