Clinical Aged Care Assessor
Job DescriptionWe are seeking a qualified professional to conduct holistic assessments with a client-centred approach, focusing on wellness and reablement.
- Conduct comprehensive assessments of clients in their homes, capturing individual information and developing support plans and unique goals.
- Support older Australians to continue living independently by considering assistive technology, social support services, and community care.
The successful candidate will work closely with clients, families, staff, and other stakeholders to ensure high-quality client outcomes. They will utilise the integrated assessment tool with My Aged Care.
