We are seeking a highly skilled professional to lead our Dementia Support Australia (DSA) service, providing tailored support to individuals with dementia transitioning from hospital settings back to their homes or into residential care. This role offers exceptional opportunities for growth and development, ideal for those looking to advance their career in a dynamic organization.
As the Team Leader, you will be responsible for ensuring that our service meets its key performance indicators while focusing on individual client needs. You will work closely with an integrated team to deliver a consistent approach to our 'Next Chapter' initiative, promoting empowerment, mentorship, and support for staff. Your leadership skills will enable you to develop a culture of excellence, driving service delivery within the case management framework and reducing the impact of behaviors on people with dementia, their carers, and staff.
The successful candidate will have qualifications in Nursing, Allied Health, or a related health degree, with experience supporting older people and carers with dementia, emphasizing psycho-social case management approaches. They will possess strong multitasking, communication, planning, and organizational skills, as well as attention to detail and analytical skills. Customer management and administrative skills are essential, along with the ability to lead teams across multiple sites and travel intra- and interstate. Preference may be given to candidates eligible for Australian Health Practitioner registration and those with qualifications or studies related to dementia.
