Job Summary

The Area Coordinator role is a vital position within our Hume community team, providing support and supervision to case managers. This dynamic opportunity enables an individual to make a real difference in the lives of older people at risk or experiencing homelessness.

Key Responsibilities:

  • Promote a non-institutional philosophy by recognizing and promoting the rights of clients.
  • Ensure consistent growth and viability of the Home Support Program (HCP).
  • Develop strong relationships with local services and networks.
  • Maintain duty of care to both clients and staff at all times.
  • Provide high-quality, comprehensive case management services.

Required Skills and Qualifications:

  • Approved tertiary qualification in Social Work, Nursing, Welfare, Community Services or Allied Health.
  • Well-developed networking skills.
  • Understanding of the aged care sector and community services.
  • Current Driver's License.
  • Experience in case management supervision and staff development.
  • Experience in service delivery in a community setting.
  • Sound understanding of the aged and community services legislative frameworks.
  • Commitment to social justice.
