Job Title: Client Care Specialist

This dynamic opportunity will enable you to provide high-quality care services to clients living in their own homes, promoting their independence and quality of life.

About the Role:
  • Assist clients to maintain their independence and lifestyle by delivering a tailored level of care
  • Address issues of social isolation for program clients and support them in maintaining or improving their accommodation
  • Conduct initial and ongoing assessments for new clients to ensure they receive the most appropriate care
  • Develop and implement personalized care plans and schedules to meet clients' unique needs
Your Skills and Qualifications:
  • A recognized tertiary qualification in Social Work, Allied Health, or Nursing
  • Experience working in mental health is desirable
  • Previous experience as a Client Care Specialist would be highly regarded
  • Mandatory skills include high-level computer, documentation, and budgeting skills
