Family Support Specialist

We are seeking a dedicated Case Manager to provide vital support to families with children in care, enhancing safety and connecting them with local community services.
About the RoleThe Early Intervention & Family Support (EIFS) program collaborates with parents and family carers who have children in their care. The EIFS Case Manager will conduct initial assessments and develop personalized family case plans. They will identify barriers and deliver practical in-home assistance for carers, oversee case management activities, provide parenting information and trauma guidance for carers, and specific supports for children to strengthen links to community and culture.
Main Responsibilities:- Conduct thorough initial screenings and assessments of family needs
- Create and implement tailored family case plans
- Identify and overcome obstacles hindering family well-being
- Provide customized, in-home support for carers
- Oversight of case management activities
- Offer parenting and trauma guidance for carers
- Safeguarding children's connection to community and culture
