Job Title: Clinical Care Coordinator
Job Description:
A highly skilled Clinical Care Coordinator is required to lead a clinical team in a medium/large aged care service. You will be working collaboratively with the leadership team to provide hands-on support, guidance and motivation to the team members. The role involves providing clinical guidance to residents and care staff, undertaking resident assessments and case conferencing, and ensuring compliance with industry standards and regulations.
Main Responsibilities:
  • Provide expert clinical guidance and advice to residents and care staff
  • Undertake comprehensive resident assessments and case conferencing
  • Accurately complete, review, and update relevant documentation
  • Motivate, train, and develop clinical team members
  • Ensure compliance with quality indicators and industry standards

Requirements:
This role requires an AHPRA Registered Nurse registration with previous leadership experience. You must also have up-to-date knowledge of achieving accreditation standards and the ability to identify and improve areas of clinical risk and quality guidelines.
