Health Liaison Officer Role">

The Health Liaison Officer will be responsible for providing liaison, guidance, education, and support to external stakeholders, assisting clients to navigate their health journey and support needs. The ideal candidate will have experience in complex care planning, preferably in a community setting, and possess excellent communication and interpersonal skills.

The role involves developing and reviewing individual health support plans for people with complex disability and healthcare needs, delivering training to provide theoretical and competency-based education to staff, and ensuring medication and health administration practices are in line with policy and procedures.

The successful applicant will have strong clinical background, current AHPRA registration, and minimum 3 years' post-qualification experience. They will also have proven experience in disability and excellent technology skills. The role requires the ability to work autonomously and travel to other regions intermittently (planned in advance).

">Key Responsibilities">
  • Developing and reviewing individual health support plans for people with complex disability and healthcare needs in consultation with the individual, their medical practitioner, and other relevant healthcare professionals.
  • Delivering training to provide theoretical and competency-based education to CPL staff in class, online, and individualized training for specific support needs; assisting with the review/creation of educational material when required.
  • Ensuring medication and health administration practices are in line with policy and procedures, professional guidelines, and quality standards.
  • Attending specialist and complex health appointments when required.
  • Providing clinical advice, resources, information, and expertise to individuals and their service delivery teams.
  • Liaising with hospital discharge teams, allied health professionals, support coordinators, and service delivery teams to ensure safe transition through significant health events.
  • Championing person-centered care practices by ensuring that clients are offered the opportunity and encouraged in a positive manner to make their own decisions and choices about their lives and lifestyle.
  • Supporting clients with informed health-related decision-making by ensuring that clients receive information on options as appropriate, and in the best way that is responsive to their individual circumstances.
  • Collaborating with the wider Health Liaison team to ensure medication and health procedures/guidelines and support plans are reflective of current best practice and can be easily understood by service delivery teams, clients, and families.

  • Current AHPRA registration with minimum 3 years' post-qualification experience.
  • A strong clinical background with experience in complex care planning, preferably in a community setting.
  • Proven experience in disability.
  • Experience facilitating group training sessions to Direct Support Workers.
  • Excellent communication and interpersonal skills.
  • Good technology skills.
  • Strong problem-solving and time management abilities.
  • Ability to work autonomously.
  • Have experience providing group training sessions.
  • A current driver's license.
  • NDIS Worker Screening Clearance (We can help).
