We are seeking an experienced Registered Nurse to join our dedicated team as a Health Liaison Officer.
The successful candidate will have a strong clinical background with experience in complex care planning, preferably in a community setting.
Key Responsibilities:- Develop and review individual health support plans for people with complex disability and healthcare needs.
- Deliver training to provide theoretical and competency-based education to support workers.
- Ensure medication administration practices align with professional guidelines and quality standards.
- Collaborate with the wider Health Liaison team to ensure best practice is reflected in support plans and procedures.
- A current AHPRA registration with minimum 3 years' post-qualification experience.
- A strong clinical background with experience in complex care planning, preferably in a community setting.
- Proven experience in disability and facilitating group training sessions.
- Excellent communication and interpersonal skills.
- Good technology skills and problem-solving abilities.
