Job Title:

Case Manager - Young People in Intensive Therapeutic Care

About this Role:

This is an exceptional opportunity to provide high-quality casework services for young people in need of intensive therapeutic care. As a Case Manager, you will be responsible for planning, implementing, and reviewing case plans to achieve permanency and identified outcomes for young people and their families.

The ideal candidate will have a strong background in social work, youth work, or a related field, with experience working with young people with complex trauma and a solid understanding of adolescent development, trauma, attachment, and resilience theory.

Key Responsibilities:
  • Deliver comprehensive and dynamic case management approaches with ongoing analysis, decision-making, and record-keeping
  • Collaborate closely with ITC Coordinators, Therapeutic Specialists, House Managers, and other ITC staff to provide a holistic therapeutic response
  • Develop and implement strategies and plans for individual young people within a trauma-informed framework

Requirements:

  • Bachelor's degree in social work, youth work, community services, or a relevant field
  • Experience working with young people with complex trauma and understanding of adolescent development, trauma, attachment, and resilience theory
  • Demonstrated skills and experience in casework practice, preferably in OOHC or child protection
