Job Title: Ndis Support Coordination Team Leader

The role of the NDIS Support Coordination Team Leader is to provide leadership and strategic direction in collaboration with the NDIS and Disability Program Manager to deliver Support Coordination services to people with disabilities in our local area.

This leader will be responsible for ensuring that Peninsula Health services are in line with the NDIS Quality and Safety Commission requirements, including preparation and completion of external audit against the NDIS Quality and Practice Standards.

Main responsibilities include:

  • Providing leadership and strategic direction in collaboration with the NDIS and Disability Program Manager
  • Overseeing financial viability, including staff and resource management, monitoring of KPI's, participant service agreements and operational efficiency and effectiveness
  • Ensuring Peninsula Health services are in line with the NDIS Quality and Safety Commission requirements
  • Partnering with external agencies to ensure positive experiences and outcomes for our participants
  • Evaluating services, including consumer experience and outcomes

The ideal candidate will have demonstrated current specialist knowledge and skills in the provision of Support Coordination, an understanding of legislation and standards relevant to the disability sector, strong proactive and collaborative leadership, and highly developed communication skills.

The successful candidate will be responsible for delivering high-quality support coordination services to individuals with disabilities. They will work closely with the NDIS and Disability Program Manager to develop and implement strategies that meet the needs of our participants.
