Care Coordinator Lead

About the Role

We're seeking a compassionate and strategic Care Coordinator Lead to oversee and coordinate the diverse health and social support services within our care zone.

Key Responsibilities:
  • Lead a multidisciplinary team of professionals working in the care zone
  • Build and maintain partnerships with internal services and external agencies to enhance access and integration
  • Ensure delivery of trauma-informed, harm reduction-based care to all service users
  • Collaborate with medical professionals to coordinate high-quality medical care and maximize billing opportunities
  • Represent and advocate for the care zone in planning and leadership forums
  • Support quality improvement initiatives and ensure compliance with policies and procedures
About You

To be successful in this role, you'll have qualifications in healthcare, social work, or a related field, as well as experience engaging with people who use drugs, particularly those with complex needs. Strong leadership skills and knowledge of harm reduction frameworks, mental health policy are essential.
