The role of Clinical Nurse Specialist in Assertive Outreach involves facilitating access to relevant services to improve the health and wellbeing of community members who have difficulty accessing appropriate healthcare.
Key responsibilities include collaborating with healthcare providers and government agencies to ensure coordinated care, working with individuals to develop personalized plans for accessing healthcare services, and providing education and support to clients on managing their health conditions and improving their overall wellbeing.
