About The Role

Tweed Community & Allied Health are seeking a Clinical Nurse Specialist to join their Assertive Outreach team is to facilitate access to relevant services to improve the health and wellbeing of community members who due to their personal life circumstances and/or reduced social economic status have difficulty accessing appropriate health care. Specifically, the target population includes individuals who are experiencing homelessness or other socioeconomic disadvantage/isolation and/or those who attend outreach services (such as soup kitchens) aimed at people who are in need of physical, social and economic support. Working in collaboration with other healthcare providers and relevant government and non government agencies to ensure that care is well coordinated and that client needs are met.
