Case Manager Opportunity

This pivotal role is designed to support individuals and families experiencing or at risk of homelessness.

As a key member of the team, you will provide tailored support, case management, and advocacy to help people access safe accommodation, essential services, and opportunities to build more stable futures.

Key Responsibilities:
  • Conduct thorough client intakes, assessments, and develop personalized case plans to meet the unique needs of each individual.
  • Provide crisis intervention, information, referrals, and ongoing support to empower clients to overcome challenges and achieve their goals.
  • Act as an advocate on behalf of clients with government agencies, courts, and service providers to ensure they receive the necessary assistance and support.
  • Engage in home visits with clients residing in temporary or transitional accommodation to assess their needs and provide guidance.
  • Facilitate connections to life skills and education programs to equip clients with the tools and knowledge necessary for long-term success.
  • Maintain accurate and up-to-date client records and case notes to inform decision-making and ensure continuity of care.
  • Create a safe, welcoming, and inclusive environment for all service users, fostering trust and promoting positive outcomes.
  • Develop and maintain partnerships with community services and stakeholders to expand resources and enhance support networks.
  • Support service delivery targets and contribute to ongoing improvement initiatives to optimize program effectiveness.
Requirements:
  • A minimum Certificate IV (or current tertiary study) in welfare, community services, or a related field.
  • Experience working with individuals who are homeless or disadvantaged, with a strong understanding of issues such as family breakdown, mental illness, addiction, and social disadvantage.
  • Knowledge of local community support services and ability to establish strong partnerships with service providers.
  • Excellent written and verbal communication skills, with the ability to engage effectively with diverse groups and stakeholders.
  • Solid computer and administrative skills, with proficiency in using relevant software and systems.
  • Ability to work independently as well as collaboratively in a team, with a strong commitment to teamwork and mutual support.
