Clinical Transformation Lead

We are seeking a clinical transformation lead to drive the delivery of safe, high-quality, person-centred care across our aged care services and key programs.

This is an exciting opportunity for a senior clinical professional to take a hands-on and strategic approach, influencing governance, driving business growth, and uplifting clinical capability through effective leadership.

Key Responsibilities:
  • Lead and oversee clinical governance, compliance, and risk management across aged care services, ensuring adherence to regulatory requirements and industry standards.
  • Provide expert clinical leadership and mentoring, including review and approval of care plans, medication management, wound care protocols, infection control, and allied health interventions.
  • Monitor and evaluate clients' health outcomes, identifying areas for improvement and implementing proactive measures to mitigate clinical risks and concerns.
  • Develop and implement the Ageing Well Model of Care and Clinical Governance Framework across our aged care services, driving quality and consistency in care delivery.
  • Contribute to strategic planning, tender preparation, and expansion into new service streams, leveraging commercial acumen and operational expertise to drive business growth.
Requirements:
  • Current Australian Health Professional Registration Authority (AHPRA) registration as a Division 1 Registered Nurse.
  • Minimum 5 years' leadership experience in the aged care sector, with a proven track record of delivering high-quality care and improving clinical outcomes.
  • Strong commercial mindset, with the ability to drive operational efficiencies and align team objectives with strategic business goals.
  • Comprehensive knowledge of person-centred care models, with a focus on patient-centred care and evidence-based practice.
  • Exceptional leadership, influencing, and problem-solving skills, with the ability to guide teams through change and inspire high performance.
  • Proactive, hands-on approach with a commitment to continuous improvement and best practice.
  • Highly experienced in Aged Care Services with deep knowledge of clinical governance and regulatory requirements.
  • Strong time management skills, including managing multiple deadlines and prioritising tasks effectively.
