Support Young People in Their Healthcare Journey

We are seeking a dedicated and empathetic individual to support young people aged 14-25 with chronic illness or disabilities through their transition from paediatric to adult healthcare systems.

This dynamic, person-centred role requires strong communication and organisational skills to effectively navigate complex care needs while managing competing priorities.

The Transition Care Coordinator will play a vital role in coordinating individualised care plans, working directly with young people, families, and healthcare providers.

About the Role:
  • Coordinate individualised care plans for young people aged 14-25 with chronic illness or disabilities
  • Work directly with young people, families, and healthcare providers to ensure seamless transitions between paediatric and adult healthcare systems
  • Attend paediatric and adult health clinics to provide ongoing support and follow-up care
  • Foster self-management skills in young people and their families

About You:

  • Empathetic and patient with the ability to build trusting relationships with young people and their families
  • Strong communication and organisational skills to manage competing priorities
  • Proactive in your learning and committed to delivering coordinated, high-quality care

Requirements:

  • Relevant qualifications in Nursing, Social Work, Occupational Therapy, Speech Pathology, Physiotherapy, or Dietetics
  • At least 3 years of clinical experience in the profession or in the clinical area of the specified post-graduate qualification
  • Current NSW Driver's Licence and willingness to travel within the Local Health District
