Community Mental Health Practitioner

A mental health professional in the community setting conducts comprehensive assessments and develops tailored support plans for individuals in acute care services.
Key Responsibilities:- Conduct thorough needs assessments of consumers and create personalized strategies for ongoing support.
- Provide high-quality interventions, including counseling and therapy, to foster positive outcomes.
- Collaborate with internal teams and external partners to ensure seamless and coordinated care delivery.
- Advocate for consumers' rights and interests, adhering to relevant legislation and guidelines.
- Support staff development and contribute to ongoing service enhancement initiatives.
- Postgraduate qualification in Social Work and eligibility for Australian Association of Social Workers membership.
- Experience working in community mental health settings, with confidence in crisis intervention, risk assessment, and triage.
- Strong communication skills and ability to work collaboratively with diverse stakeholders.
- Highly organized and proficient in using electronic medical records to manage complex caseloads.
- Contribute to delivering compassionate, person-centered care that improves the lives of at-risk individuals and their families.
