Case Manager Team Leader

Prestige Inhome Care is seeking an experienced Team Leader to lead the Home Care Package (HCP) Case Management team in delivering high-quality, client-centered care. The successful candidate will oversee daily operations, ensure compliance with care plans and annual reviews, and drive continuous improvement through team management and procedural enhancements.
Key Responsibilities- Lead and manage the Case Management team, providing direction, training, and support to ensure high performance.
- Monitor and improve team KPIs, offering coaching and feedback for continuous development.
- Ensure compliance with care plans, conduct internal audits, and provide training to maintain audit readiness.
- Oversee daily operations, managing resources efficiently and resolving complex client issues.
- Lead team-based projects, implementing process improvements and supporting change management initiatives.
- Previous Team Leader or people management experience.
- Experience in community care/home-based care services.
- Prior Experience in Case Management and navigating complex situations.
- Exceptional customer service skills.
- Excellent communication and interpersonal skills.
- Demonstrated self-awareness and self-management ability.
- Proactive and focused on continuous improvement.
