Job Title: Community Care Lead

Are you interested in permanent opportunities and available for at least 25 hours per week? Our team requires support from Community Care Leads to assist our team of PCWs working in the community. These positions are crucial to support our growth and commitment to delivering exceptional support. The roles are open to current team members and are permanent part-time or full-time positions, rostered over five days.

About the Role:

In this position, you will support our Community teams to enable clients to live their best possible life. Every day is different, and any day could involve:

  1. Supervising, mentoring and training a team of PCWs and CSA's to optimize client support.
  2. Providing ongoing support to our team, including backfilling roster shortfalls as required.
  3. Liaising with our community clients to monitor service and meet care plan requirements.
  4. Pro-active participation and communication with the Community HCP team to optimize service delivery.

About You:

To be considered for this important and rewarding work, you will have:

  1. At least two year's paid experience as a PCW or EN working in the community and be a current team member.
  2. An empathetic, patient and caring nature.
  3. A motivation to support junior colleagues.
  4. Flexibility to prioritize client requirements.
  5. Availability to work minimum 25 hours per week, Monday – Friday roster.
  6. Exceptional verbal and written communication skills.
