Job Title: Community Care Lead

We are seeking a compassionate and skilled professional to join our team as a Community Care Lead. In this role, you will have the opportunity to support individuals in their communities and make a positive impact on their lives.

About the Role:

As a Community Care Lead, you will be responsible for supervising and mentoring a team of professionals who provide support to clients in their homes. You will also be responsible for ensuring that clients receive high-quality care and services that meet their needs.

Responsibilities:

  1. Supervise and mentor a team of professionals to ensure they have the skills and knowledge needed to provide high-quality care and services.
  2. Ensure that clients receive high-quality care and services that meet their needs.
  3. Liaise with clients, families, and other healthcare professionals to ensure that clients receive comprehensive care and services.

Requirements:

To be successful in this role, you will need to have:

  • At least two years' experience working in a community care setting.
  • A strong understanding of community care principles and practices.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work a variety of shifts, including evenings and weekends.
