Key Responsibilities:
- Conduct comprehensive assessments and ongoing reviews of customer needs to ensure that their support plans are tailored to their specific requirements.
- Develop, explain, and manage service agreements and support plans in collaboration with clients, their families, and health professionals.
- Oversee financial management and casework to ensure alignment with funding guidelines and regulatory requirements.
- Collaborate with community agencies to deliver integrated services and advocate for clients to ensure that their needs are met.
- Regularly monitor and adjust support plans to address evolving needs and ensure that clients receive the best possible outcomes.
Principal Accountabilities:
- Ensure that support plans are developed in partnership with customers and their representatives, taking into account their preferences, needs, and goals.
- Monitor and review support plans regularly to ensure that they remain relevant and effective, and make adjustments as needed.
- Deliver services in accordance with person-centered and consumer-directed principles, prioritizing the needs and preferences of clients.
- Assist clients in accessing alternative services when current services no longer meet their needs, ensuring a seamless transition and minimal disruption.
- Liaise with community-based agencies and service providers to ensure a cohesive approach to service delivery and adherence to program objectives.
- Provide advocacy for clients as needed, ensuring that their rights and interests are protected.
- Monitor clients' budgets in accordance with program guidelines, ensuring that resources are allocated effectively and efficiently.
- Represent the organization in external forums and meetings, promoting the values and mission of Payworx.
- Engage in personal professional development, staying up-to-date with industry trends and best practices to ensure that services are delivered to the highest standard.
- Ensure compliance with organizational policies, statutory and contractual requirements related to service delivery, and maintain accurate records and documentation.
- Manage communication with key stakeholders both internally and externally, ensuring that information is shared effectively and efficiently.
- Uphold principles of equity and anti-discrimination in the workplace, promoting a culture of inclusivity and respect.
Requirements:
- Relevant qualifications in Community Services, Human Services, Nursing, or a related field.
- Proven experience in Case Management and supervising staff, with a strong track record of delivering high-quality services.
- Exceptional communication, organizational, and administrative skills, with the ability to work effectively in a fast-paced environment.
- Flexibility and strong coordination abilities to manage diverse services effectively, prioritizing the needs of clients and stakeholders.
- Current Criminal Record Check, Working with Children Check (if applicable), and Driver's License.
Preferred Qualifications:
- Experience in health/aged care or community services, with a strong understanding of the sector and its challenges.
- Background in Quality Improvement Programs, with a focus on delivering high-quality services and improving outcomes.
- Availability for on-call support, with the ability to work flexibly and respond to changing client needs.
- Previous customer service coordination experience in the health/aged care, non-profit, or community services sector, with a strong understanding of the needs and preferences of clients.
