About the Role

The Clinical Facilitator and Operational Support Specialist will play a pivotal role in ensuring high-quality, safe, and person-centered care and services are delivered to residents and clients.

This hands-on leadership position involves working closely with organisational services managers/teams to monitor clinical/care outcomes, respond to risks, and lead change.

Key Responsibilities
  • Monitor Southern Cross Care's clinical care and medication management software to inform clinical/care decisions.
  • Leverage expertise to improve ACQSC readiness across homes and services.
  • Provide support and advice to Service Managers on complex resident/client needs, ensuring alignment with ACQSC standards.
  • Develop and implement solutions to address systems/documentation gaps related to clinical/care needs.
  • Analyse issues to create timely plans of action, driving improvements in resident/client outcomes.
  • Deliver formal/informal mentoring and educational support to staff to address identified knowledge gaps.
  • Foster a positive learning culture, promoting engagement and excellence within Southern Cross Care.
Requirements
  • Bachelor of Nursing or equivalent, including current AHPRA registration as a Registered Nurse.
  • Demonstrated experience in a Clinical Nurse or Quality role.
  • Excellent understanding of ACQSC standards, expectations, and outcomes.
  • Proven ability to lead staff through periods of cultural, service, and approach changes.
  • Leadership experience in direct care, incorporating Healthy Ageing principles.
  • Strong interpersonal skills, with clear communication and problem-solving abilities.
  • Effective negotiation and decision-making skills.
  • Demonstrated competence in adult learning principles.
