Job Description

Our organisation provides settlement solutions for the Australian community. We work with refugees and migrants to empower them to participate fully in their community.

The Support Coordinator role plays a key part in empowering NDIS participants to access chosen supports, enabling them to participate more fully in their community. They collaborate with other agencies to provide holistic support, information and referral services to clients through a Care Team Model.

This role focuses on providing high-quality Support Coordination, facilitating the implementation of all supports identified in a participant's plan. This includes managing a caseload, identifying and implementing innovative service delivery strategies, and developing relevant planning tools and practices.

Key Responsibilities:

  • Managing a caseload of people with a disability, purchasers, families and networks in accordance with relevant legislation and professional practice guidelines.
  • Identifying and implementing innovative service delivery strategies that empower participants to exercise choice and control over their supports.
  • Developing and using relevant planning tools and practices to enhance service provision, including recording progress towards client goals.
  • Maintaining current knowledge and providing expert advice on changes in NDIS regulations.
  • Implementing, monitoring and reviewing the effectiveness of participant supports and negotiating agreed/funded supports as appropriate.

Required Skills and Qualifications:

  • A minimum of 1-2 years' experience in case management with the human services or disability sector in Australia.
  • Relevant qualifications such as a Diploma in Community Services or Social Work.
