We seek a compassionate, resilient and purpose-driven Therapeutic Case Manager to champion the wellbeing of children and young people in our foster care program. As a Therapeutic Case Manager, you will work closely with children, carers and families to provide specialist case management, co-create therapeutic care plans, lead transitions, and drive cultural safety and inclusivity. You will also contribute to service development, continuous practice improvement, and reporting. Key Responsibilities:

  • Provide specialist case management to children and young people with complex needs.
  • Co-create therapeutic care plans with carers, Therapeutic Specialists & multi-disciplinary teams.
  • Lead transitions with empathy and focus on long-term stability and permanency.
  • Support, guide, and reflect with foster carers managing high-needs cases.
  • Drive cultural safety and inclusivity for Aboriginal, Torres Strait Islander and CALD children.
  • Build collaborative relationships with internal and external stakeholders.

Required Skills and Qualifications:

  • A heart for trauma-informed, child-centred work.
  • Relevant qualifications in Social Work, Psychology, Youth Work or similar.
  • Experience working with children, carers, and families in complex care environments.
  • Strong understanding of child protection, therapeutic models, and cultural competence.

Selection Criteria:

  • Appropriate tertiary qualifications.
  • Professional casework skills, including knowledge of LAC and CIRSSP/CRIS.
  • Sound professional knowledge and theory base.
  • Ability to participate collaboratively and constructively within teams of peers, staff, and other stakeholders, as well as initiative and effective personal judgement when working alone.
