Multidisciplinary Health Team Lead

**Team Environment:**
We are seeking a motivated and experienced Multidisciplinary Health Team Lead to join our dynamic health team. As a key member of our team, you will play a critical role in providing high-quality social work services to clients in the community setting.
The successful candidate will possess extensive clinical experience across a diverse range of diagnoses and conditions, with a strong understanding of the current AASW Practice Standards. You will be responsible for delivering and monitoring social work services, working closely with interdisciplinary colleagues to ensure effective service provision.
Key Responsibilities:
- Develop and implement case plans, liaise with other health professionals, and advocate for access to appropriate community services.
- Work closely with senior managers to develop and deliver proactive social work services.
- Supervise, train, and perform performance appraisals of Level 1/2 social workers within the service.
