Job OverviewAs a Case Manager, you will provide case management support to individuals in both community and residential settings, assisting them to achieve housing stability, experience improved social inclusion, and attain self-mastery to sustain tenancy and lead fulfilling lives.
This role involves:
- Establishing effective relationships with clients that encourage self-determination and participation.
- Providing case management support to individuals in accordance with TSA case management models and standards of practice.
- Conducting thorough assessments, inclusive of needs, risks, and vulnerabilities.
- Ongoing assessment of the individual's presenting issues, vulnerabilities, and needs from initial intake to exit.
- Developing individual case plans in collaboration with clients, including a range of interventions that address client needs or goals, as well as barriers to achieving those.
- Assisting clients to access support services and establish community supports, and where necessary, advocating with service providers.
To succeed in this role, you will require:
- Tertiary qualification in Community Services, Social Work, or a related field. A degree-level qualification is preferred.
- Relevant experience in a social service environment, including client assessment and case management.
- A Working with Children Check.
- Driver's Licence.
- A willingness to undergo a National Police Check.
