Job Overview

As a Case Manager, you will provide case management support to individuals in both community and residential settings, assisting them to achieve housing stability, experience improved social inclusion, and attain self-mastery to sustain tenancy and lead fulfilling lives.

This role involves:

  • Establishing effective relationships with clients that encourage self-determination and participation.
  • Providing case management support to individuals in accordance with TSA case management models and standards of practice.
  • Conducting thorough assessments, inclusive of needs, risks, and vulnerabilities.
  • Ongoing assessment of the individual's presenting issues, vulnerabilities, and needs from initial intake to exit.
  • Developing individual case plans in collaboration with clients, including a range of interventions that address client needs or goals, as well as barriers to achieving those.
  • Assisting clients to access support services and establish community supports, and where necessary, advocating with service providers.
Key Requirements

To succeed in this role, you will require:

  • Tertiary qualification in Community Services, Social Work, or a related field. A degree-level qualification is preferred.
  • Relevant experience in a social service environment, including client assessment and case management.
  • A Working with Children Check.
  • Driver's Licence.
  • A willingness to undergo a National Police Check.
