About the Role:

The Community Geriatrics service is a geriatrician-led multidisciplinary team providing care to older people with complex medical, functional, and psychosocial needs in their own homes. Our team offers short-term case management to prevent hospital admissions through delivering care in the home environment.

Key Responsibilities:

  • Provide individualized case management, care planning, and liaison for older patients in the community
  • Deliver direct, expert clinical care, selecting and implementing different therapeutic interventions
  • Develop effective complex discharge plans/hospital avoidance through education, equipment, and referral, including client/program evaluation
  • Link clients and carers with relevant community resources and services
  • Communicate with clients' family, carers, and other service providers
  • Contribute to an integrated team approach and culture responsive to client needs

Requirements:

  • For RN3: Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia, holding a current practicing certificate. Minimum 3 years post-registration experience in geriatrics and community nursing practice.
  • For AHP3: Hold an appropriate degree-level qualification and membership (or eligibility) with the relevant professional association, and/or registration with the relevant Board – Physiotherapy, Occupational Therapy, Speech Pathology, Dietetics, Psychology, Social Work, or Exercise Physiology. Demonstrate ability to assume responsibility for clinical allied health services to a designated area as part of an interdisciplinary team.
