We are seeking a skilled Community Outreach Coordinator to join our team and make a meaningful impact in the lives of our community members. Our centre is a not-for-profit, registered charitable organisation that serves as a hub for the community.

The successful candidate will provide client-centred support through strengths-based intake and assessment, case management, and connecting clients with community-based groups, activities, services and supports.

  • Manage comprehensive client assessments and case management, ensuring all interactions are planned and structured to improve health, connection, and well-being outcomes.
  • Maintain accurate and comprehensive records, including reports for various stakeholders.

Key Responsibilities:

  • Client Services:
    • Provide client-centred support through strengths-based intake and assessment, case management, and connecting clients with community-based groups, activities, services and supports.
    • Manage comprehensive client assessments and case management, ensuring all interactions are planned and structured to improve health, connection, and well-being outcomes.
    • Maintain accurate and comprehensive records, including reports for various stakeholders.
  • Community and Strategic Engagement:
    • Engage actively with the community, promoting social inclusion and belonging.
    • Develop and maintain relationships with internal and external stakeholders, including medical professionals, government agencies, and community organisations.

Selection Criteria:

  • Tertiary degree in Social Work, Psychology, Human Services, Public Health/Health Promotion, or a related field.
  • Minimum of two years of relevant community work experience.
  • Strong understanding of evidence-based frameworks including person-centred and trauma-informed approaches.
  • Demonstrated ability to work with disadvantaged individuals or those facing significant barriers.

Desirable Skills:

  • Knowledge of local agencies and services.
  • Familiarity with social prescribing.
  • Experience in managing databases.
