Physiotherapist - Community-Based Allied Health Service

We are seeking a highly motivated Physiotherapist to join our large multidisciplinary team in Melbourne. As a community-based physiotherapist under our in-Home Care (IHC) team, you will be responsible for clinical care to consumers in a community setting.
Key Responsibilities- Participate as a key member of Health and Wellbeing staff providing a service that reflects the Baptcare Vision, Mission and Values.
- Assessment and planning, and the delivery of safe and effective care that is best practice, tailored to individual needs and optimises health and wellbeing of the consumer.
- Ensure own clinical practice is within scope of regulatory guidelines including the Aged Care Quality Standards and support consumers to live the life that they chose.
- Demonstrate excellent communication skills and a positive consumer focused profile to all stakeholders.
- Compliance to all Work, Health, and Safety responsibilities.
- Effective management of high-impact or high-prevalence risks associated with the care of consumers.
- Minimisation of infection-related risks through implementing standard transmission-based precautions to prevent and control infection.
- Contributes pro-actively to the continuous improvement process within the program.
- Demonstrated ability to listen to in a way that accepts cultural and other diversity and in ways that maintains and promotes a consumer's dignity and respect and understand what is important to them and support them to achieve their goals.
- Demonstrated experience in providing Physiotherapy services within the Community/Aged or Disability sector that gives each consumer safe and effective clinical care that is best practice, tailored to individual needs and optimises health and wellbeing.
- Assessment and planning, including consideration of risks to the consumer's health and well-being, informs the delivery of safe and effective care and services.
- Demonstrated ability to seek information to support evidence-based practice.
- Advanced skills in the creation of timely and thorough consumer records including documentation of care provided to meet all statutory requirements and collecting and maintaining accurate data and statistics as required.
- Proven ability to understand and work within relevant legislation and guidelines including those set by the Physiotherapy Board or Australia.
- Highly organised and adaptable to work in a rapidly changing environment and a consumer's changing circumstances or preferences.
- Demonstrated ability to make timely decisions and problem solve to ensure quality consumer outcomes.
