Physiotherapist - Community Care Specialist

We are seeking a highly motivated Physiotherapist to join our multidisciplinary team in Melbourne. As a community-based physiotherapist under our In-Home Care (IHC) team, you will be responsible for delivering high-quality, best-practice care to consumers in a community setting.
Key Responsibilities- Assess and plan care for consumers, ensuring safe and effective delivery of services that meet their individual needs and optimize their health and wellbeing.
- Work collaboratively with other healthcare professionals to provide comprehensive care and support to consumers.
- Develop and implement individualized care plans that address the unique needs and goals of each consumer.
- Conduct regular assessments and reviews to ensure the ongoing effectiveness of care plans and make adjustments as needed.
- Communicate effectively with consumers, families, and other healthcare professionals to ensure seamless care coordination and delivery.
- Demonstrated experience in providing physiotherapy services in a community or aged care setting.
- Strong assessment and planning skills, with the ability to develop and implement individualized care plans.
- Excellent communication and interpersonal skills, with the ability to work effectively with consumers, families, and other healthcare professionals.
- Ability to work independently and as part of a multidisciplinary team.
- Current registration as a Physiotherapist with AHPRA and a current driver's license.
