Occupational Therapist

We are seeking a highly motivated Occupational Therapist to join our large multidisciplinary team in Melbourne. As a community-based Occupational Therapist in Home Care (IHC), you will be responsible for delivering high-quality, best-practice care that meets the expectations of consumers and demonstrates a commitment to maintaining an optimal level of dignity, respect, comfort, and quality of life.

• Provide professional, quality occupational therapy care to meet the individual needs of consumers.
• Engage consumers in the development, delivery, and evaluation of care and services, and support them in that engagement.
• Ensure that all consumers receive regular assessment of their individual needs and develop a plan to meet these needs.
• Take appropriate action in the event of changes in the consumer's needs, including referral to other health professionals as required.
• Effectively manage high-impact or high-prevalence risks associated with the care of consumers.
• Minimise infection-related risks through implementing standard transmission-based precautions to prevent and control infection.
• Report consumer risks to management, including inappropriate behaviours or actions not aligned to our values or the Aged Care Quality Standards.
• Ensure that the scope of practice is compliant with regulatory guidelines and Baptcare policy and procedures.
