Job Description:
We are seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will be responsible for providing case management services to individuals living with a disability and/or mental illness.
Your primary duties will include:
- Developing and implementing individual support plans that align with clients' service agreements and support needs;
- Liaising with external agencies to ensure the safe implementation of care plan goals;
- Providing one-on-one client-facing work, including medication assistance and basic personal care;
- Assisting in new client sign-ups by providing relevant information to prospective clients;
- Participating in project work as directed by senior management;
- Maintaining accurate and timely documentation in a confidential manner.
Required Skills and Qualifications:
To be successful in this role, you will need:
- Tertiary qualifications in a relevant field or equivalent experience within the community services sector;
- Effective interpersonal and communication skills, including the ability to work with diverse client groups;
- Strong problem-solving skills, particularly in complex cases;
- Ability to work independently and as part of a team;
- Accurate and timely decision-making skills;
- A solid understanding of technology systems, including Outlook, Excel, and mobile applications.
