Job Title: Support Worker

 

About This Position

This is a leading provider of community health services seeking a compassionate and dedicated Support Worker to join our team. The role involves delivering personalized support to help older adult clients maintain and grow their independence.

As a Support Worker, you will have the opportunity to make a real difference in the lives of older adults. You will be responsible for implementing individual service plans, healthcare plans, and positive behaviour support strategies to promote well-being and quality of life.

Your Responsibilities

  • Deliver personalized support to help older adult clients maintain and grow their independence
  • Implement individual service plans, healthcare plans, and positive behaviour support strategies
  • Offer companionship, engaging conversation, and emotional support while assisting with personal care needs
  • Assist clients with transportation to important appointments

Key Requirements

  • Cert III Qualification in Aged Care or Individual Support (with completed aged care components)
  • Experience working with older adults
  • Ability to follow and implement individual service plans, healthcare, and positive behaviour support plans
