CareChoice is a leading NDIS and TAC/WorkSafe registered disability provider, delivering a wide range of services and providing complex support across the Disability sector. We are seeking two full-time NDIS Support Coordinators to join our team at our head office in Mulgrave.

As a NDIS Support Coordinator with CareChoice, you will be responsible for assisting Participants with complex needs to build personal capacity to make decisions and choices about the implementation of all supports in their NDIS plan. You will focus on addressing barriers and reducing complexity in the support environment, while also assisting Participants to ensure they are on track to achieve their goals as articulated in their NDIS plan or Care Plans.

A day in the life as a NDIS Support Coordinator with CareChoice may see you supporting our Participants with:

  • Support Coordination to assist in implementing their NDIS plan.
  • Coordinate services in Participants' NDIS plans including the management of complex multi-disciplinary teams.
  • Provide advice and consultation with Participants, their families, and other stakeholders in response to complex and challenging situations.
  • Develop action plans following consultation with Participants, their families, and other stakeholders to resolve barriers arising from complexity.
  • Liaise and collaborate with key stakeholders to establish mutual goals of coordinating service delivery for Participants.

To be successful in this role, you will have greater than 1 year's experience with providing quality care services, implementing NDIS plans and building rapport with Participants in an outcome-driven position in the Disability, Youth Welfare, Child Protection and/or community services sector.

Ideally, you will have:

  • Relevant tertiary qualifications in an industry-related discipline.
  • Comprehensive understanding of the NDIS and Disability Act.
  • Well-developed interpersonal, negotiation, and complaints resolution skills.
  • Strong networking and relationship management skills.
  • Sound understanding of SDA.

Qualifications required:

  • NDIS Worker Screening Clearance
  • Working with Children Check (Employee type)
  • Valid Victorian Driver's Licence
