Job Title: Community Care Specialist

Job Summary:

The Flinders And Upper North Local Health Network is seeking a skilled Community Care Specialist to join our team. As a Community Care Specialist, you will play a vital role in providing high-quality care and support to individuals in their own homes or in residential aged care facilities.

Key Responsibilities:

  • Provide personal care, cleaning, meal preparation, transport, and social support to individuals who are aged, frail, or disabled.
  • Work collaboratively with consumers, their families, and other health professionals to provide quality outcomes and maintain consumer independence.
  • Support consumers in identifying and setting goals, and in determining what level of involvement they want in managing their own care arrangements.
  • Provide services in accordance with the Consumer Directed Care model, which allows consumers to maintain greater control over their lives.
  • Work autonomously in the community, with flexibility and adaptability to meet the changing needs of consumers.

Requirements:

  • Qualifications and experience in a relevant field, such as aged care, disability, or healthcare.
  • Ability to work collaboratively with consumers, families, and other health professionals.
  • Strong communication and interpersonal skills.
  • Flexibility and adaptability to meet the changing needs of consumers.
  • Ability to work autonomously in the community.
