Job Description

As a Compassionate Home Care Specialist, you will play a vital role in delivering high-quality care to patients who require complex medical treatment in the comfort of their own homes. This position offers the opportunity to work both autonomously and collaboratively with healthcare professionals to deliver exceptional patient-focused care.

You will be responsible for conducting clinical assessments, administering treatments, and managing patient care. Your expertise in wound care, IV therapy, and CVC management will enable you to develop and implement comprehensive care plans tailored to each patient's needs.

In this senior clinical position, you will also provide education to patients and families on health management and recovery, while upholding the highest standards of professionalism and compassion. You will possess current registration as a Registered Nurse, proven experience in acute and community nursing, and excellent communication and documentation skills.

Responsibilities include:

  • Conducting thorough clinical assessments to identify patient needs
  • Developing and implementing individualized care plans
  • Administering treatments and managing patient care
  • Providing education to patients and families on health management and recovery
Required Skills and Qualifications

To succeed in this role, you will need:

  • Current AHPRA registration as a Registered Nurse
  • Proven experience in acute and community nursing
  • Expertise in wound care, IV therapy, and CVC management
  • Excellent communication and documentation skills
