Compassionate Home Health Care Specialist

We are seeking a compassionate and skilled Registered Nurse with expertise in home health care to deliver high-quality patient care.
The ideal candidate will possess a strong clinical background, excellent communication skills, and a passion for helping patients achieve optimal health outcomes.
Key Responsibilities:
- Conduct comprehensive patient assessments in the home setting to evaluate the patient's physical and emotional well-being.
- Develop and implement individualized care plans based on physician orders, patient goals, and evidence-based nursing practices.
- Administer medications, injections, wound care, and other treatments as prescribed by the physician.
- Provide patient and caregiver education on disease management, medication usage, home safety, and recovery techniques.
- Coordinate care with other healthcare providers, including physicians and therapists, to ensure continuity of care.
Requirements:
- Current and unrestricted RN license in Australia or equivalent.
- Strong clinical skills and ability to perform evidence-based nursing practices.
- 1+ years of clinical experience in a healthcare setting, preferably in home health, hospice, or community health.
- Excellent communication and interpersonal skills with the ability to work effectively and independently with patients, families, and healthcare providers.
