About the Role

We are seeking a highly skilled Clinical Care Coordinator to join our multidisciplinary team. As a Clinical Care Coordinator, you will play a critical role in delivering high-quality evidence-based clinical care to adults experiencing mental health concerns and their families and carers.

Key Responsibilities
  • Conduct initial assessments and service navigation supports to ensure individuals receive the best possible care.
  • Conduct risk assessments, including assessment of suicide risk, violence/aggression, and vulnerability, and provide risk management in collaboration with the individual, their family/carers, and other support systems.
  • Run groups and support engagement with the service to promote recovery and well-being.
Requirements
  • Must have one of the following: Full AHPRA registration as an Occupational Therapist, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist, or AASW registration/eligibility as a Social Worker.
  • Knowledge and understanding of the theory underpinning evidence-based interventions and clinical practice, as well as demonstrated excellence in the practical application of such, in the areas of mental health and alcohol and drug use.
  • Highly developed communication (including written) and interpersonal skills to engage with people, advocate on their behalf, and establish and maintain effective relationships with staff, clients, families, and friends, and stakeholders.
  • Experience in the use of electronic client/customer management systems, ability to use a desktop computer, and experience with Microsoft Office applications.
  • Must be eligible to apply/currently hold a Working With Children Check (WWCC) and NDIS worker screening clearance.
