Clinical Care Manager

The Clinical Care Manager will play a pivotal role in the implementation and delivery of support and maintenance for clients receiving Home Care Packages funded by the Commonwealth Government. This will involve comprehensive assessment, monitoring, and case management within a professional framework, as well as coordinating delivery and brokerage of services within the package budget.
This role offers a unique blend of computer-based care work and clinical care, allowing the successful candidate to demonstrate holistic care at its best. The Clinical Care Manager will work closely with clients, families, and other service providers to ensure the highest level of care and support.
Key Responsibilities- Conduct comprehensive client assessments and develop care plans to support clients at home
- Provide advanced holistic care skills within a Case Management framework
- Demonstrate an understanding of the target population and their needs, from care coordination to complex case management
- Manage budgets within a CDC framework
- Work within the required legal framework underpinning the Home Care Packages Programme
- Take professional responsibility for care recipients' interface with the service system, including advocacy
- Participate in quality improvement processes, including case review and team meetings
- Deliver a high level of specialized knowledge and skills in their identified area of specialty
We are seeking a highly skilled and experienced Clinical Care Manager to join our team. The ideal candidate will have:
- A Bachelor of Nursing/RN with experience highly desirable or recent RN graduate with previous nursing care and/or care plans, budgets, and case management experience
- APHRA registration essential
- Relevant Case Management and aged care experience in a similar role
- Highly developed assessment skills and the ability to creatively problem solve complex care situations
- Relevant understanding and experience in the delivery of health services and/or community-based services as well as complex case management
- Proven ability to contribute to and work as part of a dynamic team in addition to operating independently
- Demonstrated skills in networking, liaison, and the development and maintenance of strong links with relevant service providers and users
- Valid working rights in Australia
