Bolton Clarke is seeking a skilled Clinical Care Manager to join our team. As a Clinical Care Manager, you will be responsible for managing the implementation and delivery of support and maintenance for clients receiving Home Care Packages funded by the Commonwealth Government.
Key Responsibilities- Provide comprehensive client assessment and development of care plans to support clients at home.
- Deliver advanced holistic care skills within a Case Management framework.
- Demonstrate expertise in budget management within a CDC framework.
- Take professional responsibility for care recipients' interface with the service system, including advocacy.
- Participate in quality improvement processes, including case review and team meetings.
- Bachelor of Nursing/RN with experience highly desirable or recent RN graduate with previous nursing care and/or care plans, budgets, and case management experience.
- APHRA registration essential.
- Relevant Case Management and aged care experience in a similar role.
- Highly developed assessment skills and the ability to creatively problem solve complex care situations.
- Competitive base salary + $15,900 Salary packaging + $2,650 meal and entertainment allowance.
