Clinical Care Coordinator Role

As a Clinical Care Coordinator, you will play a pivotal role in shaping exceptional community care. You will oversee complex client care plans, ensuring our team is trained and assessed in advanced clinical skills.

  • Provide clinical guidance and continuous improvement to ensure quality, compliant services.
  • Oversee complex care plans, medication administration, and clinical interventions (including wound care, catheter care, stoma care, palliative care, and more).
  • Deliver training, coaching, and skills assessment for Home Care Workers.
  • Manage incident and risk processes and ensure timely resolution.
  • Assess and support customers living with dementia or cognitive impairment.
  • Conduct falls risk assessments and implement prevention strategies.
  • Collaborate with healthcare professionals, clients, and families to ensure best-practice outcomes.
