Clinical Care Coordinator

Embark on a fulfilling role as a Clinical Care Coordinator, providing exceptional community care to individuals in need.

About the Role:

This is an opportunity for a skilled Registered Nurse to lead clinical services and ensure quality care is delivered to our clients.

  • Develop and implement complex care plans, medication administration strategies, and clinical interventions to meet individual needs.
  • Provide leadership, coaching, and skills assessment to Home Care Workers to enhance their capabilities.
  • Maintain incident and risk processes to ensure timely resolution and minimize harm.
  • Conduct assessments and support customers living with dementia or cognitive impairment, and implement prevention strategies to promote independence.
  • Collaborate with healthcare professionals, clients, and families to deliver best-practice outcomes.

Your Qualifications and Experience:

  • Current AHPRA registration as a Registered Nurse.
  • Evidence of ongoing professional development to stay up-to-date with industry trends and regulations.
  • Minimum 3 years' experience in aged care or community care, with a proven track record of leadership and staff development.
  • Strong knowledge of Aged Care Quality Standards and relevant legislation to guide clinical decision-making.
  • Experience in complex clinical assessments and care planning to meet individual client needs.
  • Proficiency in Microsoft Office (Word, Excel, Outlook) to manage administrative tasks efficiently.
