Job Opportunity

The role of Support Coordinator involves ensuring seamless coordination of support services for clients and assisting them in achieving their goals.

This position requires engaging with service providers, monitoring progress toward plan goals, and assisting customers to identify community or mainstream supports that can help achieve set plan goals.

Responsibilities:
  • Coordinate support services and monitor progress toward plan goals by engaging with service providers and allied health professionals.
  • Assist customers in identifying community or mainstream supports that align with their individual goals.
  • Develop programs that promote skill development and encourage participation in leisure activities.
  • Promote connections and networking opportunities outside the service system for clients to access.
  • Help build capacity around understanding plans and their implementation for customers.
  • Establish and maintain effective links with NDIS, TLC programs, and other provider services.
Qualifications:
  • Ability to engage with the community to create meaningful relationships.
  • Experience applying best practice within organizations and maintaining integrity around legislation and NDIS guidelines.
  • Experience coordinating complex and potentially at-risk client caseloads.
  • Ability to develop programs promoting skill development and community involvement.
  • Strong collaboration with NDIS and related services.
