Disability Support Professional
We are seeking a dedicated and compassionate individual to provide day-to-day leadership and direction to teams of frontline staff in our Forensic and Specialist Transitions service.
About the RoleThe successful candidate will be responsible for mentoring and inspiring frontline staff to deliver high-quality services that promote independence, community connections, and meaningful impact for individuals with complex psychosocial and forensic disability needs.
Key Responsibilities- Support Coordination: Coordinate responsive supports for participants with complex psychosocial and forensic disability needs.
- Needs Assessment: Conduct regular reviews of supports to maintain up-to-date knowledge of each participant's changing needs.
- Relationship Building: Act as the first point of contact for participants and staff, fostering strong relationships and promoting a culture of respect and empathy.
- Home Visits: Carry out regular home visits to ensure participant goals and needs are being met.
- Service Quality Assurance: Conduct regular service quality and safety audits to uphold standards and ensure compliance with regulatory requirements.
- Record Keeping: Ensure participant records are accurate and maintained in a timely manner.
