Mental Health Care Navigator

Are you passionate about delivering high-quality mental health services to children under 12?
This is an exciting opportunity to join our multidisciplinary team as a Care Navigator for the Head to Health Kids Hub.
You will be responsible for providing comprehensive, tailored support and interventions based on evidence and validated assessment tools.
As part of our team, you will work collaboratively with medical professionals, allied health staff, psychologists, and early childhood development experts to deliver bio-psycho-socio-cultural assessments and interventions.
The successful candidate will have excellent communication and interpersonal skills, with the ability to build strong relationships with families and stakeholders.
We are looking for a motivated and dedicated individual who is committed to improving early intervention outcomes and making a positive impact on the lives of children and their families.

Key Responsibilities- Develop and implement care plans in collaboration with families and multidisciplinary teams.
- Provide referrals and liaise with schools and agencies to ensure seamless service delivery.
- Assist in developing systems, processes, and tools for consistent engagement, referral, and transition.
- Coordinate Multidisciplinary Team (MDT) meetings and align goals between families and MDT.
- Consult on collaborative information portals and create individualized referral pathways for neurodiverse individuals.
To be successful in this role, you will need:
Current work rights in Australia or permanent residency/citizenship.
Excellent communication and interpersonal skills.
A commitment to delivering high-quality mental health services to children under 12.
A willingness to work collaboratively as part of a multidisciplinary team.
