The Role- Triage and assess need for face-to-face appointments.
- Act as an autonomous clinician, assessing patients, creating treatment plans and referring them to specialists when needed.
- Be compliant with Professional Conduct guidelines and national standards.
- Conduct patient histories, physical examinations and use clinical judgement to identify potential diagnoses.
- Refer patients for investigations and specialist services when necessary.
- Determine and carry out treatment, including prescribing medication.
- Make onward referrals to other services as required.
- Ensure continuity of care.
- Work independently and collaboratively within a healthcare team.
- Provide leadership and ensure each patient receives best practice treatment.
This role is suitable for experienced clinicians who can work autonomously in Primary Care settings.
