Advanced Nurse Practitioner

We are seeking an experienced clinician to drive healthcare services and reduce hospital access.

The Role:

  • Triage and assessment of need for face-to-face appointments.
  • Autonomous clinician responsible for patient assessment, treatment plans, and referrals to specialist services.
  • Compliance with professional conduct guidelines and national standards.
  • Take comprehensive patient histories, perform physical examinations, and use clinical judgment to identify potential diagnoses.
  • Refer patients for investigations and specialist services as necessary.
  • Independently decide on and carry out treatment, including prescribing medicines.
  • Make onward referrals to other services and ensure continuity of care.
  • Work independently and collaboratively within the healthcare team.
  • Provide leadership and ensure patient care is based on best practice.
