Job Title: Primary Care Nurse Specialist
About the Role:We are seeking an experienced and skilled nurse to join our primary care team as a Primary Care Nurse Specialist. This is an exciting opportunity for a dedicated and compassionate individual who is passionate about delivering high-quality patient-centered care.
Key Responsibilities:- To assess, plan, develop, implement, and evaluate care programs to improve health and well-being of patients.
- To provide holistic, evidence-based care to patients with complex needs.
- To work collaboratively with other healthcare professionals to ensure comprehensive care and effective communication.
- To prioritize health issues and intervene in emergency situations, including initiating emergency care when necessary.
- To educate patients, families, and carers to promote self-management and empower them to take control of their health.
- NMC registration and current practicing certificate.
- Qualification in spirometry interpretation (ARTP registered).
- Experience in managing long-term conditions such as asthma and COPD.
- Basic Life Support (BLS) training and IT skills (MS Office, Systmone, EMIS).
