Job Title: Clinical Care Leader

We are seeking an experienced Registered Nurse with several years of aged care experience to join our team as a Clinical Care Leader.

This is an exciting opportunity to provide advanced clinical support and leadership for our care team at our 117 bed facility.

Key Responsibilities:

  • Coordinating the completion of baseline assessments for residents' needs and recording findings within a Positive Wellbeing Model.
  • Ensuring care plans are developed and reviewed based on assessment findings, in accordance with organisational timelines.
  • Coordinating the collection of accurate clinical data for timely submission of Aged Care Funding Instrument.
  • Maintaining resident documentation accurately reflecting care provisions and adhering to organisational requirements.
  • Coordinating external service provider care provision.
  • Reviewing and maintaining clinical programs and initiatives.
  • Collecting clinical indicator data according to organisational reporting timelines.

Requirements:

  • Current AHPRA registration as a Registered Nurse.
  • Experience in managing and leading teams in the aged care sector.
  • Strong knowledge of the aged care accreditation system and compliance.
  • Ability to educate, mentor, and motivate staff.
  • Excellent communication skills, both written and verbal.
  • National Police Check and ability to obtain National Personal Insolvency Check.
